Coordinate with Councilor Allia Acosta for your schedule. You may reach her at 09052804339. 3. Using your official email account send [email protected] the following information: a. Org president b. Contact number c. Adviser 4. Upon compliance with item 3, a soft copy of the accreditation forms will be sent to your organization which you will fill up electronically, print after completion and submit to OSA together with the gender sensitivity training certificate. 5. Send the accomplished forms to the same email address for archiving purposes. 6. Wait for the results of the assessment. Kindly disseminate. Thank you.
