**Very Important Information Regarding the New Health Care Reform** By October 1, 2013, all employers must provide notices to their employees, whether they are part time or full time, explaining their health care options. These notices should also include information about federal and state health insurance marketplaces. Failure to provide these notices to your employees will result in a fine of $100.00 per day, per employee. There are 2 model notices that the Department of Labor has provided that can be used. Both are attached to this email via PDF files. If you are an employer who PROVIDES insurance, please fill out the form titled, Coverage Offered. On this form you must: • Complete items 3-12 on page 2 • Answer whether or not you provide coverage to all employees or some • If you only provide to some, you must list eligible employees. Example: Full-time employees only. • Answer whether or not you offer coverage to eligible dependents. This is not asking what you pay, it is simply asking if you offer coverage. • To check the last box, you must obtain information from your insurance agent as to whether or not you are meeting the minimum value standard • You do not have to complete Page 3 • Provide completed copies of the notices to each employee before October 1st, 2013. • Have the employee sign off verifying they have recieved a copy of the notice. If you are an employer who DOES NOT provide insurance, please fill out the form titled, No Coverage Offered. On this form you must: • Complete items 3-12 on page 2 • Provide completed copies of the notices to each employee before October 1st, 2013. • Have the employee sign off verifying they have recieved a copy of the notice.
