Last in at 6:15 PM was a wrist contusion from a dropped dresser, r/o fracture - turned out to be a simple hematoma, Left Flank Pain and renal colic, with candida balanitis. Gastritis with Nausea/Vomiting, and GI bleed, a positive hemoccult with melena - treated with PPIs, H2 Blockers, x2 antibiotics, an anti-spasmodic, and anti-emetic, advised to see GI ASAP for Endoscopy (r/o erosive gastritis vs Ulcer and Bleeding), or go to ER. Influenza Vaccine Prophylaxis x2. Employer Drug Screening. A Community Clinic Join-up, here for labs, med refills, and a DOT physical. Pharyngitis, Sinusitis, Bronchospasm, Reactive Airway x5. Pharyngitis x3. Otitis x2 one required ear canal soaking with PDX solution (providone, dexamethasone, xylocaine 2%) for otalgia. Migraines x2, one more of a tension HA, the other for 1 month with persistant temporal pain waxing and waning with Steroids, and Left sided paraesthesias - sent for MRI of head and neck to r/o Mass, vs nerve root impingement. Follow up repack of MRSA Abscess - slightly improved. Male with Blurred vision, HA, Torticollis, fatigue, and elevated Blood Pressure in patient weaning themselves off Hypertensive meds against medical advise. Female with COPD post Pneumonia and hospitalization, is fine now that she is back on her meds. Denial is NOT a river in Egypt. A couple back pains. A knee & ankle pain with varus rotation of hip, & scoliosis - purely structure and function. Onychomycosis and overgrown nail plates on first and second toe - excised. A few med refills, and patient assistance. A good day. Charts done. Headed home.
