4600 Forms: We have recently had a lot of members signing off 4600 forms in Labor Relations when they receive disciplinary penalties. Everyone needs to understand you are signing away your rights to union representation and rights to a grievance when you do this. If you sign a 4600 you can’t come back a week later and request a grievance, you give that right up by signing the company form. If you have questions ask your committeeman. That’s their job to represent you.
