60 year old woman with long standing RA presented with this swelling on the ulnar side of her wrist just proximal to the radio carpal joint at the anatomical location of distal radio ulnar joint. The distal radio ulnar joint is separated from radio carpal or wrist joint by triangular fibrocartilage. Proximal part of the joint capsule is lax and projects between radius and ulna as a sacciform recess. Sonographic examination by scanning proximal to the wrist showed heterogeneous distension of distal radio ulnar joint and sacciform recess from isolated flare of rheumatoid arthritis at her left distal radio ulnar joint
