A 31 yrs old man presented with substernal burning, indigestion & and vague chest pain since few hours. The 1st ECG was taken by family physician and 2nd ECG was taken after few minutes in ER. The ECG shows ST elevation in inferior leads with reciprocal ST depression in AVL, subtle ST depression in V1-3. It appears to be acute IWMI. Patient was taken for emergency coronary angiogram But what a Surprise ... ! The CAG shows thrombotic lesion in LAD(Fig 3 & 4) Can anybody explain the ECG changes of IWMI and LAD lesion in CAG ?
