A 44 year old woman has had increasing difficulty swallowing liquids and solids for the past 6 months. On phy exam her fingers have reduced mobility because of taut nondeforming skin. Barium swallow shows marked dilatation of esophagus with beaking indistal portion where there is marked distal narrowing. Biopsy specimen from lower esophagus shows prominent submucosal fibrosiswith little inflammation.
