A 70 y old indian male with h/o DM on glimipiride 2 mg Metformin 500 MG bd Amlodipine 5 MG od Bisoprolol 5 MG od Presented with acute onset of cough with frothy blood tinged sputum n SOB. Bp in resus was 198/100 with a heart rate of 110. B/L crackle were heard. ABG showed hypoxia n hypocapnia. ECG showed LV strain pattern. Initial CXR showed hilar congestion with bat wing shadows. Was given lasix n tridil infusion n cpap. Pt improved for day 1 n then continue to deteriorate. His recent 2 X rays re shown. Crp is 9 . High. WBC 12000 TROP HIGH - 2.5. RFT - normal. Corona n H1N1 - negative. Culture negatives. Urine - no RBC or casts. Vasculitic work up pending. Being given piperacillin tazobactum along with clarithromycin. Also hydrocortisone 50 q8h. Need your expert opinion.
