A combination of “human error” and “lack of leadership” - TopicsExpress

A combination of “human error” and “lack of leadership” caused USS Guardian to run aground Tubbataha Reef near Palawan last January, the U.S. Navy said. In a statement, U.S. Navy’s Pacific Fleet said the tragic mishap could have been mitigated had USS Guardian leadership and watch teams adhered to prudent and safe navigation principles. “This tragic mishap was wholly preventable and was the product of poor voyage planning, poor execution, and unfortunate circumstances,” the 160-page report stated. “This investigation uncovers no single point of failure. Instead, there were numerous links in the error chain leading up to the grounding,” it added. U.S. Pacific Fleet commander Admiral Cecil Haney said lack of leadership led to the watch team’s disregard of visual cues, electronic cues, and alarms in the hours leading up to the grounding.ph.news.yahoo/u-s--navy-admits--human-error--in-tubbataha-incident-095505047.html
