A link to SETMA’s PC-MH Senior Medical Student Externship and to eight other documents and series about PC-MH setma/senior-medical-student-externship/SETMAs-MS4-Patient-Center-Medical-Home-Selective-Syllabus August 23, 2013 As we arrive at the last day of Summer’s time with SETMA, we are all a little sad. Summer’s winsomeness has endeared her to all of us, but the personal emotional bond has been overshadowed by her remarkable clinical knowledge and ability. From my perspective, Summer’s experience has been beneficial in the following ways: 1. She has seen a “learning organization” actively learning. 2. She has seen an integrated team functioning generatively and collegially. Her experience has included primary care physicians, specialists and nurse practitioners, administrators and staff. She has experienced the reality that NPs are NOT “mid levels” but genuine, excellent and highly respected colleagues. 3. She has seen how the “business of medicine” is critical to the support of excellence in the science and practice of medicine. 4. She has seen the power of auditing and analytics in population management and quality improvement in care. 5. She has seen the dynamic and continuous development of electronic patient management tools which facilitate and necessitate excellence in care. 6. She has learned a about Patient-Centered Medical Home including patient activation, patient engagement, shared decision making and patient centered conversations. She has been exposed to tools of PC-MH such as HCAHPS and CAHPS. And, she has seen PC-MH in action. She saw a physician with 40 years of practice have a eureka moment when key elements of PC-MH because real and true before his eyes. 7. She has seen practicing providers at all stages of their careers engaged in education and training. 8. She has seen an intuitively and dynamic EMR in action. She knew of a meeting which took place on 8.21.13, which resulted in the following observation about SETMA’s IT deployment: “…there are many things that should be said about the success of yesterday’s meeting; however, I only want to comment of one. And that was the your opening presentation on the capabilities of your care management system. It was inspirational. Your under- standing of ‘life in the exam room’, coding, claims, regulatory issues, quality standards, human behavior, engagement, etc. etc. was impressive and extraordinary. I truly found myself saying – ‘this guy has already fully delivered on the promise of EMRs and coordinated care that so many other only aspire’. Such leaps forward only happen with leadership, and you have delivered. It was wonderful watching you in action. Well done.” 9. She has seen how the practice of medicine is fun and how providers can enjoy one another and can enjoy their patients. 10. She has seen a group of people with a celebratory and restless spirit – celebratory because they constantly acknowledge improvement and restless in that they are always eager to do better. As an alumnus of the San Antonio School of Medicine with four other colleagues who are also graduates, I am proud of my school for many reasons, Summer has given me another. I am amazed at how much she knows already. Her compassion and sincerely are engaging. Her analytical skills are suburb. She will mature into an outstanding physician. She has a standing invitation to intervene for a career with SETMA or for a recommendation and encouragement if she makes another choice. Perhaps my wife summarized it best with her comment to Summer at lunch yesterday. She said, “If we can ever help in any way, call us. If you just want someone to talk to call me.” Summer is now a part of our family and those are more than just words to SETMA. James (Larry) Holly, M.D. C.E.O.
