ANNOUNCEMENT Bicol University College of Science DEPARTMENT OF CHEMISTRY FOR 1ST SEMESTER ENROLLMENT AND START OF CLASSES SY 2014-2015 1. Enrollment dates: A. Freshmen a. May 19-20 – qualified freshmen b. May 21 – waitlisted Available slots will be determined after the enrolment of qualified applicants. c. May 22 – qualified but below quota (QBQ)/transferees/shifters Available slots will be determined after the enrolment of waitlisted applicants. For QBQ, only those with Composite Rating of 85% or higher will be entertained. Ranking will be based on Composite Rating, High School Math and Science Grades and English Plus Rating. Submit photocopy of BUCET Result and High School report card for ranking purposes. For Shifters and Transferees, please have your record/certification of grades assessed at the BU Office of Admissions before proceeding to Department of Chemistry. Please see the BUCS Registrars Office for the necessary form to be accomplished. For Qualified, Waitlisted, QBQ and Transferees, the following requirements are to be submitted upon enrolment: BUCET Result English Plus Result (if necessary) Interview Result (if necessary) High School Report Card (F-138), (F-137 not allowed) Medical Certificate issued only by the Bicol University Physician Certificate of Good Moral Character issued by the HS Principal/Guidance Counselor 1 pc. 2”x2” picture 1 pc. 1”x1” picture 1 pc. Long brown envelope 1 pc. Long RED folder, hard Clear photocopy of authenticated (preferably NSO Birth Certificate) B. Returning Students a. May 26-30 – higher year level students b. June 2-6 – Late enrolment with fine Requirement: Signed Clearance for 2nd Semester 2013-2014 or Summer 2014 2. Venues - Advising of students – Department of Chemistry Faculty Room - Enrollment/Assessment – CSB3 Lecture Room - Payment of enrollment fees – Cashier’s Office at CSB1 (priority number to be given by the Cashier) - Issuance of COR – Registrar’s Office at CSB1 Important: Only the enrolling students are allowed within the enrollment areas. 3. All fees required by the CSC and CBO’s shall have been paid by the student prior to issuance of COR by the Registrar. 4. Partial Payment Requirements: Accomplished Partial Payment form from Dean’s Office Barangay Clearance Water/Electricity Bill (March or April 2014) Application form needs to be signed by the parents/guardian and endorsed by concerned department chair prior to submission to Dean’s Office 5. June 9, 2014 – start of classes - Returning students, whether enrolled or yet to enroll, shall be in prescribed school uniform. Students not in prescribed uniform shall not be allowed entry to any of the buildings in the College of Science 6.
