Admission Procedures - Admission Billing • The doctor wants you to be admitted for observation. • The doctor would like you to stay in the hospital for observation. • The doctor says that you will have to stay in the hospital for observation. • The doctor says that you will have to be admitted to the hospital tonight. • You are being admitted for observation by Dr. Chatchai. • You’ll probably have to stay in the hospital for three days. • This is a consent form to place yourself under our care. Please read this paragraph and sign here. • Could you please fill out this admission form? • You can indicate on the form how you wish to pay, by cash, credit card, or through insurance coverage. • We accept cash and all major credit cards. Personal Cheques can also be accepted with proper identification. • It is very important that you identify your insurance company at the time of admission. This is to allow enough time for processing your claim. Within a day or two after requesting approval, your nurse will inform you of the result (approved or denied). The insurance co-ordinator (ext…?) or the in-patient cashier (ext…?) can help you understand your insurance benefit prior to your admission and will gladly explain any policies you are unsure about. • Do you have the policy with you? • What kind of room would you prefer, a single or a twin? • Do you wish to have a single room or a VIP room? • Let me check if we have a single / a twin room available. • I’ll check our room availability. • We have a single reserved for you. • The single room is 2,500 baht per day and a VIP room is 3,750 baht, plus a 300 baht nursing service charge and 300 baht for 3 meals. • Your doctor has ordered a special diet for you. • The cost will be added to your bill, automatically. • The doctor’s fee and treatment costs are separate. • The admitting specialist / consultant visits will be charged at Bht.200/250/300/450 per visit, depending on specialist / consultant attending. If you are admitted under a community based specialist, we recommend that you discuss this fee with your treating doctor. • Local calls can be made free of charge, whereas calls between the emirates, mobile, and international calls are added to your hospital bill with a surcharge. • During your stay we provide a therapeutic diet for you, hospital gowns, basic toiletries (tooth brush, soap, shampoo, conditioner, towels), reading material (daily newspapers, magazines), neonatal top feeds (if required), meals for your attendants, consumables (disposable items, diapers, sanitary pads, etc) at an additional cost. • In addition, we encourage you to bring along any items that will enhance the comfort of your stay such as your personal sleepwear, robes, slippers, reading and writing material, personal toiletries, hair care accessories/products, and for our maternity patients an outfit and a receiving blanket for the baby on the day of discharge, and a camera. • Do refrain from bringing valuable items, as the hospital will not be responsible for stolen or missing items. • Alternatively, you can deposit your valuables with the Security Department free of charge and collect them upon your discharge. You should receive a receipt which should be kept in a safe area or with a relative. Kindly inform your nurse to make proper arrangements. • For information about hospital fees you can inquire at the Finance Department. • Your room is 702 on the seventh floor. The nurse aide will show you to your room. • I would like to take you up to your room now. Please come with me. • The nurse aide will take you up to your room.
