Attention Current Medicaid Clients: URGENT ATTENTION NEEDED. Dear Palms Family, We know that the new Medicaid policies can be confusing and frustrating. Our staff is here to support you in selecting a plan and assuring the transition is smooth and successful (just like labor.) There are three ways to secure your plan so there is no interruption to care. Option 1: Call 877-711-3662 The Medicaid Choice Counseling Hotline and press 1 for English on the menu, and then 1 again for enrollment. You will need you Medicaid Gold-Card number, the address listed on your medicaid file, date of birth, social security number and the name of the plan you would like to be enrolled in (we are contracted with Humana.) The representative will walk you through the steps. Option 2: You can choose your plan on the internet. Visit https://flmedicaidmanagedcare/enroll/#PinSignOn You will need the Pin number assigned to you in the letter sent directly to you by the State of Florida or if you did not receive letter with PIN, call our office for the information you will need. (medicaid gold card and billing ID) Follow the prompts on the website and choose your plan. We are contracted with Humana. Option 3: You may also assign our billing specialist to be your representative to choose a plan (we will choose the plan we are contracted with, Humana.) For this option simply let us know when you check in for your next visit so that we may assist you with the paperwork. All our clients must complete the process by June 10, 2014. It is imperative that you participate in the process so that we can continue to receive reimbursement for the care we provide. Any client not enrolled in an HMO by June 30th will be discharged from care and referred to the health department as The Palms will be unable to be reimbursed. Thank you for your time and attention.
