Attention Harlem Health Plan Members (Excluding Bus Drivers): The Rockford Health Systems VIP Wellness Screening Clinic has moved to a new location (next door to the former location)! The new address for the clinic is: 2350 North Rockton Suite, 308 - Building 2 (the building next door to the former location, and the tallest building between the hospital and the clinic). Signage has been placed throughout the RHS campus directing users to the new facility. Parking for this building is still in the lot on the West side of the clinic. The parking lot is located directly behind the Building 2 clinic. Park in the west lot, come in the entrance, take the elevator to the 3rd floor, and the clinic is located in Suite 308. The phone number for scheduling appointments remains: 815-971-2180. Please remember: The deadline for screenings to be completed is August 31, 2013. There will be no additional screenings scheduled this year, so please be sure to make your appointment early enough that you get a time that works for you!
