Bank Support Worker- 51 Rutland Gardens MAIN PURPOSE To enable the tenants to lead a valued and fulfilling life and to maximise the potential capacity of all tenants, physically, intellectually, emotionally and socially, whilst working in accordance with CMG’s philosophy of care, it’s Policies and Procedures and all relevant legislation. KEY RESPONSIBILITIES Monitors individual tenants’ general health and well being on a daily basis and to reports any concerns to the manager. Serves as a keyworker to individual tenants, and when required liaises with relevant others, including relatives and healthcare professionals. Assists in the effective compilation of individual reviews, support plans and risk assessments and takes an active part in support plan meetings and reviews. Carries out tasks and activities identified in the support plan to meet tenants’ needs. Maintains accurate records, in line with legislation and the Company’s policies and procedures. Provides appropriate information and support to enable tenants to make informed choices. Supports tenants to maintain good budgeting skills. Supports tenants to maintain contact with their family and/or friends and promotes positive links with the local community and the facilities therein. Encourages and supports tenants to participate in the cleaning and general upkeep of their flat. After appropriate training, supports tenants unable to self-medicate with the administration of medication. Offers support and guidance, to tenants able or wishing to self medicate. Responds appropriately to challenging incidents. Ensure all communication is clear both verbally and in writing and that all incidents are documented accurately and in accordance with the Company’s policies and procedures. Maintains a working knowledge of, and complies with, Company Policies, Procedures and Guidelines.
