Bid Proposal Specifications For Snow Plowing/Ice Removal 2013-2104 Our Lady of Lourdes 318 Plane St. Weatherly, PA 18255-1012 (570) 427-4123 All contractors must provide insurance certificates with $2 million liability coverage. This can be in a single limit of $2 million or a combination of $1 million general liability with a $1 million Excess Liability. The contractor must also list on the insurance certificate worker compensation insurance for their employees. The insurance certificate must list the following additional insured status: The Diocese of Allentown Bishop John O. Barres Our Lady of Lourdes Church Rev. Floyd Caesar, Jr. These certificates must accompany the proposal. Contractor and/or drivers must secure a current valid Pennsylvania Driver’s License. The contract will include November 1, 2013 through April 30, 2014. The contract will include the parish rectory, church, garage and all sidewalks (including the handicapped ramp) on the premises. The contract bid (flat rate) will be for each snow/ice event of 2” or more. Contractor will be responsible for all equipment, labor (shoveling sidewalks) and supplies. Materials used on the concrete (sidewalks/handicapped ramp) should consist of a low-calcium, earth friendly material to prevent damage to concrete. Proposals must be submitted in a sealed envelope on or before Monday, October 21st, 2013 by 2:00 p.m. to the parish office at the above address. There will be a special meeting to open the bids on October 24th.
