Bone union is not just dependent upon mechanical factors. The healing tissues require an adequate blood supply. Therefore, when the injury has led to significant soft-tissue damage, union is likely to be slow. At the time of surgery, care must be taken to minimise disruption to the blood supply of the healing tissues. Smoking has been shown to delay fracture healing. Non-steroidal anti-inflammatory medications also seem to delay fracture healing; they are therefore best avoided in situations in which union is likely to be slow.
