Family Self Sufficiency Case Manager – Harrisburg/Dauphin County area - The FSS Case Manager is responsible for the provision of the Family Self-Sufficiency Case Management services in the Harrisburg/Dauphin County area. Responsibilities include, but are not limited to: providing intake and assessment services, and linking individuals to resources and supports; recruiting and sustaining program participants to ensure caseload of 30 active clients; administering a comprehensive family needs assessment that enables individuals and families to determine their goals; providing ongoing monitoring of each individual’s progress towards goals; empowering individuals to achieve self sufficiency, utilizing incentives and other program tools to encourage ongoing participation in programming; conducting individual or group education sessions (one per quarter) on self sufficiency topics (may include: budgeting, financial management, housing, etc.); ability and willingness to complete home visits; and collecting, inputting, tracking, and maintaining client and case management records in written and electronic records and files, utilizing both agency and regulatory systems. Minimum qualifications include a Bachelor’s degree (preferred) with emphasis in human services related field and/or social work and case management experience; public speaking skills; ability to present information and train individuals and groups on educational/self-sufficiency topics; working knowledge of computers including Microsoft Office products, and a valid PA driver’s license with access to properly insured and reliable transportation, with an ability to drive in the three county service area Community Action. The candidate must be able to obtain Act 33/34 clearances upon hire. Day to day will require travel within Dauphin County. The salary is $27,500, with competitive benefits offered.
