As part of Anglicare, our South West Sydney Commonwealth Respite and Carelink Centre contributes to the support and maintenance of caring relationships between carers and their dependent family members or friends. The centre works by facilitating access to information, respite care and other support or assistance about community, aged care, disability, youth, mental health and other support services. THE ROLE In this position, you will be responsible for providing excellent customer service to external and internal clients by responding to phone calls effectively, assessing and conducting care plans with the client in accessing emergency and short-term respite service, coordinating for the booking of respite services using brokerage funds and liaising with different service providers. You will also be completing general administration duties including data entry into Carelink and Client Manager, and will be working independently and as part of a team. You will also be assessing clients to determine if referrals to other services are appropriate. We currently have two positions available: • Care Coordinator – Full-time fixed term (maternity leave contract) until October 2015 • Care Coordinator – Full-time fixed term until June 2015 SELECTION CRITERIA To be considered for this position, it is essential you have: • Post-secondary or tertiary qualifications and experience in welfare, social work, disability, health or a related area – Minimum a Cert. 4 in Community Services or related field • Experience working with carers • Broad knowledge of services available in the community to support carers especially in the South West Sydney Region • Sound knowledge of the issues relating to the provision of community care services and an understanding of the needs of carers and their care recipient • Ability to assess client needs and work with clients to develop care plans to meet their needs • Demonstrated ability to work co-operatively within a multidisciplinary team environment • Experience in handling inquiries and dealing with clients over the phone • Computer literacy - Ability to use Microsoft Office applications • Experience in data entry & good case notes skills • Good organisational skills. • Excellent communication skills both verbal and written • Excellent telephone skills • Current NSW Driver’s license Your application will also be highly regarded if you posses the following: • Experience in the coordination of brokerage services and the ability to do multi tasks under pressure • Ability to utilise word processing and database programs • Previous experience administering brokerage funds. • Counselling experience. • Previous experience with respite care and working within a tight deadline • Demonstrate an understanding of assessment and case management skills BENEFITS Anglicare will offer generous salary packaging options, a friendly working environment and a commitment to professional development.
