Clinical Pearl 11 Developmental delay.... Moderate developmental delay resulting from prenatal or perinatal insults is obvious by 3 months of age, though it may be suspected earlier in life. Milder forms of delay may be apparent much later. Every infant must be evaluated at 3 months of age by rapid screening. Thus, if an infant has a normal development at 3 months of age, normality may be nearly assured. Mild variation in achieving age appropriate milestones may be acceptable and so also lagging behind in a single milestone. However, careful evaluation is mandatory. We need to be aware of abnormal development. Early head control may denote spasticity. Cooing is not a guarantee or normal hearing. Failure to recognise strangers by 6-7 months of age is abnormal. If speech is not developed by 18 months of age, relevant investigations for the delay are mandatory.
