Club Filipino Members who received the proxy form must present it and get the official ballot pink copy on July 26, whole day at the office and also on July 27 morning. 2. If you received the proxy form and will not vote on the 27th, you may send the proxy form copy w/ authorized signature on page 2 and 3 together with the ID number.
