Context therapy : a new intervention approach for children with cerebral palsy What is CP(cerebral palsy ) is non progressive injury to immature brain leading to motor dysfunction accompanied by disturbances of perception , cognition , communication and behavior . The context therapy is physiologic intervention depends on using the dynamic system theory . The dynamic system posits that motor behaviors are organized around functional tasks that influenced by three sources : child characteristics , task demands and environmental influences . The context therapy emphasizing changing the parameters of the task or environmental rather than focus on child impairments . we designed the intervention based on the the three sources by three steps : goal identification , assessment and intervention strategies . The procedure are : 1) first step for the pt was to do (COMP)*measurement it identify activity that represent problem in ADL for each infant , after the physical therapist took every problem activity and set goals based on the problem that led to the limitation in activity daily level . * what is COMP ? is an individualized measure which parents identified up to five priority issues for their child in their areas of self care , productivity and leisure . To identify activity we ask the parents to focus on the activity that the child was trying to do or interested to do but was having difficulty performing . Based on COMP the therapist and the parents put the goals . 2) pt videotaped the children and saw how they used stuff as help in achieving each goal , then the PT took factors that can be change or adequate to help in achieving the goals quickly . 3) they increase the number of sessions 4) all this included PT and parents works at team 5) every child had one physical therapy to interact and to communicate with the family . The conclusion : 1) the article suggested that physical therapists trained to change the functional and environmental instead of the child constrain . 2) the context therapy were similar to the child focused approach which emphasized physical therapy rehabilitation of body structure (muscle strength,balance and range of motion). Example for the context therapy : one child had difficulty independently stepping onto her bus because of the height of bus . Instead of working on improving the childs quadriceps muscle strength to climb the bus, the physiotherapist called the administrators school district and explained the problem and requested a bus with steps that where less . The school complied and the next day the child achieved his goal to get onto her school independently .
