Customer Checklist - Application for Certification The following information is required to apply for a Certification according to ASME A17.7/ CSA B44.7: Elevator Systems 1. 2. According to ASME A17.7/CSA B44.7 Mandatory Appendi x I-4 your application shall include the following: A written declaration that the same application has not been submitted to any other AECO, or a written explanation as to why the application is being subm itted to another AECO. Name and address of the applicant / authorized repr esentative and the place of manufacture. Elevator Systems 3. 3.1. a) description of the equipment b) a description of a particular elevator system, su b-system, component, or function to which this Code is being applied Technical documentation: explanation as to why the application is being subm itted to another AECO. Code Compliance Document which includes the followi ng: being applied c) a deviation list of ASME A17.1/CSA B44 Parts, Sec tions, or requirements that have been addressed by compliance with ASME A17.7/CSA B44.7 d) technical documentation to demonstrate conformity a nd enable verification of conformance e) a list of applicable GESRs f) the risk assessment report with name and expertise of team members and the date of completion of f) the risk assessment report with name and expertise of team members and the date of completion of the risk assessment processes g) procedures for acceptance inspections and tests to verify conformance with the CCD h) procedures for tests, periodic inspections, maint enance, replacements, adjustments, and repairs to b e incorporated into and made a part of the Maintenanc e Control Program (MCP) required by ASME A17.1/CSA B44, requirement 8.6.1.2.1 3.2.
