Dear Friends. I will be meeting Chief Coroner Dirk Huyer this June 26 with regard to the totally inept death investigation of Terra almost eight years ago. I really need your assistance. Please e-mail Chief Coroner Dirk Huyer at the e-mails below demanding that a thorough death investigation be initiated via a public inquest. This Office take the citizens of Ontario to be stupid and so gullible that we will swallow every thing they stated! Please have you face book and e-mails contact to do the same. Some day you may find yourself in the same situation as I have been in. [email protected] [email protected] It’s a pretty sad day in Ontario when the father of the deceased has to conduct his own death investigation based on the medical evidence and facts contained within the hospital records supported and verified by qualified surgeons located outside of Canada. Ontario citizens thought our tax dollars paid for this service; we were wrong! SHAME There are really two issues that must be addressed: 1. the totally inept death investigation by the Chief Coroner’s Office of Terra Dawn Kilby! 2. and how to prevent this office from abusing the authority it has been given by the people of Ontario? --this must be exposed publicly to ensure that it will not occur again! We, the people of Ontario need to be reassured that this Office should remain operating as is “To Speak For the Dead to Protect the Living”. Should this not occur within this Office, Legislation must be enacted in one of the following two ways: a. All personnel employed by the Chief Coroner’s Office MUST not have any affiliation whatsoever with the any College of Physicians and Surgeons, nor any Ontario Hospital nor any Ontario University Faculty of Medicine. b. Should this fail; Legislation must be passed to employ in the executive positions only those who comply with the above even if it means these people come from outside of Ontario and outside of Canada. This is to ensure true transparency and accountability through the Office of the Chief Coroner for the present and future citizens of Ontario. How could this office stated that the Standards of Care were met and that there were no systemic issues when they totally ignored so many relevant issues. See my Blog for full story. But just one issue: --- Open abdominal surgery to remove a tumor and a colon resection done without the mandatory antibiotic prophylaxis --- No antibiotics given when the 8 t0 10 inch abdominal incision became infected oozing foul, purulent liquid --- no antibiotics given when test results indicated the presence of many gram negative bacilli, in same category as C-dificile. NO ANTIBIOTICS GIVEN AT ALL DURING THE ENTIRE HOSPITAL STAY. And this Office didnt notice this or comment on this--- How do you spell cover-up? When will they actually speak for the Dead to Protect the Living. This surgeon has numerous notches on his scalpel after the death of my daughter and has been protected by the Chief Coroners Office, the College, HPARB, the DIOC, and the Health Minister. Please Help. Send a copy to the Ombudsmans Office as well as I have asked them to investigate the Chief Coroners Office for the many failed death investigations into deaths associated with hospitals/surgeons.
