Dear Parent/Guardian, Students participating in any school sports program in the 2014-2015 school year are required to have a physical completed within the past year on file at the school. These physicals need to be completed before your child begins practice. Physicals will be provided at Red Hill Junior Senior High School on March 28th and Sumner Attendance Center on April 3rd. There is no cost for the physicals. The physical examinations will be provided by the Lawrence County Memorial Hospital Primary Care Clinic. The District offers these physicals as a service and parents are under no obligation to have their child’s physical done at school. If preferred, you are welcome to have the physical completed by your child’s personal health care provider. In order for your child to have their physical at school, you must complete a consent form and fill out the general information, health history and sign the physical form. These can be picked up at the school office by you or your student. If your child will be in 6th or 9th grade next year a physical form was mailed to your residence. Please complete the required information on the physical form that was mailed out earlier. The consent form and the highlighted health history section of the physical form must be completed and returned to the school by March 24th. Please feel free to call with any questions.
