Dear friends and colleagues, so many of you have asked me about the California letter (Postgraduate Training Authorization Letter) or aka PTAL, first of all, this is a process very similar (if not worse!) than the ECFMG verification process, you do not need to take any extra testing, just yr current USMLEs, however, no program in California will even look at your application if you dont have this letter, as its name implies, you can be eligible for residency training in California only if you have the PTAL, it takes several months to get the MBC (Medical Board of California) to approve it, and that if you dont have missing courses in your clinical rotations when you were in college (usually it is 4 weeks of Family medicine and 4 weeks of Psychiatry) , you will be forced to get them done here in an ACGME accredited program (MBC will show you the programs that you can contact), or you can go back to your school in your country and finish them, whichever you prefer, personally, I think getting those courses done here will give an excellent push to your CV as it will be backed up by an eval, a LOR for an ACGME accredited PD, and at least 4 weeks of externship in the selected programs, after you get approved then you can apply to California residency.
