Differences Among Thyroid Disorders: Hyperthyroidism, Hypothyroidism, and Goiter 1. Hyperthyroidism - excess Thyroid hormones producing extremely high metabolic rates manifesting INCREASED vital signs, hunger, muscle wasting, perspiration, and irritability (Nsg Mngt: Provide rest periods and proper diet for client -- AVOID IODINE-rich food because iodine increases hormone production. Encourage root crops and berries for these carry low amount of Iodine; administer ANTITHYROIDS as ordered; must MONITOR BMR and VITAL SIGNS; Lugols Solution which corrects Iodine levels must be administered with a STRAW to avoid STAINING of teeth; if taste not tolerated, combine with a JUICE) 2. Hypothyroidism - opposite of the abovementioned. DECREASED vitals signs, et al. (Nsg Mngt: Provide activities tolerated by client. May include food HIGH in IODINE. Administer THYROID HORMONES as ordered.) 3. Goiter - enlargement of thyroid due to insufficient iodine (do not be confused with the pathophysiology of Hypothyroidism -- this doesnt show swelling most of the time; this is more on hormones, not iodine) (Nsg Mngt: AVOID GOITROGENIC food such as crops planted in high altitude areas like cabbage; must be able to manage AIRWAY PRECAUTIONS) Brunner, 2010
