ENROLLED NURSES, RETIRED PROFESSIONAL NURSES AND RETIRED ENROLLED NURSES (One year Contract renewable for 3years) Job Reference Number CBHS002/2013 Job Title: ENROLLED NURSES, RETIRED PROFESSIONAL NURSES AND RETIRED ENROLLED NURSES (One year Contract renewable for 3years) Department: HEALTH AND SOCIAL DEVELOPMENT Division: Health Services Section: Community Based Health Services Program Number of Positions Available: 1 Application Closing Date: 11/9/2013 Remuneration: R120 000 per annum Appointment requirements: Request for expression of interest from enrolled nurses, retired professional nurses and retired enrolled nurses, to manage the clinical and operational aspects of the community based health services in a defined geographic area and working as part of a dynamic interdisciplinary team and is responsible for the planning, coordination, implementation and evaluation of community and household services Appointment requirements Relevant qualification in Nursing Registered with Nursing Council as Professional Nurse or Enrolled Nurse Experience in Primary Health Personal attributes / competencies: Good communication and interpersonal skills Good team worker Duties: Manages the work activities and the operational aspects of the Community Health Workers Promotes and facilitates individual and focused care utilizing a delivery system emphasizing health promotion, disease prevention and rehabilitation principles Ensures that services are carried out in accordance with established policies and protocols, Ensure that all house-holds health status are assessed and captured. Ensures the combination of all data from the CHWs in one document. Reviews household data on a daily/weekly basis with the CHWs at Staff Meetings. Develops intervention plans with each Community Health Worker and assigns tasks based on health and social challenges identified. Facilitates , coordination and scheduling of Community Health Workers for the provision of a safe and effective household care, ensuring prompt, professional responses to referrals for services, and prompt completion of clinical documentation Do household visits where indicated and screens and examines individuals as required in their homes or at the base Interfaces with other community services needed to accomplish a family’s goals and collaborates with all sectors in order to ensure coordination of trans disciplinary and multi-sectoral interventions to families and communities Collaborates with personnel and other community providers to develop and provide a continuum of services necessary to promote program and client success. Identifies community resources to strengthen interventions Serves as a resource for clinical practice to health sciences students and coordinates and creates opportunities for students to learn and improve their skills Conducts community workshops on specific priority conditions Serves as patient/family advocate to facilitate care, communication and resolution of problems and ethical dilemmas. Ensures accurate, effective and pertinent communication with all staff and partners Provides effective and timely feedback to team and programme supervisor regarding staff compliance to standards of practice. Assists and collaborates with team in formulating, monitoring and evaluating quality of care in the application of standards of care, policies, procedures and protocols. Resolve day-to-day barriers that prevent the family or individual’s advancement along the continuum of care.
