Ebola is a wake up call to all of mankind and it must be considered a Real War for the preservation of life and the security of humanity. The challenge in this war is that like all war plans it will require the generals and soldiers who understand the battle well enough to create a comprehensive battle plan. The greatest risk of spreading the disease is not so much in the epic centers where it started but from those who enter into them and leave without controls. I am asking that the CDC and the support groups and agencies all set up a comprehensive and centralized transition and quarantine station. This station should require an eight week quarantine for all healthcare workers leaving assignments from the care of infected patients or areas. This quarantine center will be equipped for immediate care procedures and constant evaluation for the clearance period. It has been obvious that the greatest risk will be exposure and this must be as I have stated considered the highest management care procedure of every nation. It is not a procedure that can be taken lightly and a global plan must be in place for immediate isolation centers as a precaution for exposure management. In Africa; roads have been closed and entire locations restricted as affected areas are identified. It is a prudent procedure as a containment initiative that is in place. However travel monitoring in these regions must be taken seriously as it falls in and under my emphasis of exposure management or isolation. As researches globally continue to look for and to investigate treatments and possible cures we must keep the fact that isolation is currently the best prevention. In this situation it is better to error on the side of caution and wisdom as the disease still has nuances that are yet to be identified. Healthcare workers who are entering the country are advised to ensure that you prepare your immune systems before the assignment. I would call this period of preparation your resolve to study isolation methods while treating patients and to monitor and keep daily record of each and every activity of your day. This period of preparation again should be eight weeks. Solid dietary practice and an evaluation of your immune system should be considered standard. I am referring to this as the eight and eight procedure for entry and exit. It is far better to establish a departure quarantine system than it is to deal with exposure variables once healthcare workers return to their home countries. This is a war we can and must win and it takes more than just throwing money at it. It takes the strategic use of these funds to ensure our first line of defense Exposure Management has considered all possible variables. Please pass this forward as it is intended as the first advisory of its kind from The African Call Institute. This is not a let’s just help scenario, it is a situation of far reaching implication. Please push the awareness under what we call the big four. 1. Exposure Management which serves as the baseline for isolation and risk exposure. This isolation must be done for those infected and the quarantine is and should be required for all those leaving and entering the areas of isolation, care or exposure. 2. Treatment, which includes protective gear management, location and procedures for the management of the sick as well as the equipment factors associated with it. We have a hypothesis being formulated in treatment evolving around temperature, blood management and possible cryogenic strategies to manage the clock for extended time for viable care or a cure. 3. Intelligence Bank; This system should serve as the global medical log on a case by cases basis addressing every aspect of the infected party to sort characteristics that will and can serve in the reverse medical management of accumulated data. It also gives access to real time data thus allowing researchers to look at and track symptoms, responses and other valuable aspects of the disease in a statistical manner 4. Cremation of infected patients and animals must be a requirement as part of the Exposure Management procedures. It is identified here as the most significant key element. The discard of all materials used in the treatment must be an absolute discard program. Liners, gloves and all materials where vapors, bodily fluids etc. are in contact with or exposed must be under the strictest monitoring and management. Exposure as I have stated as the subject of this advisory is the risk we are fighting. I implore all agencies to focus on the logistical management of dealing with the Ebola Virus along with the treatment, management and efforts to cure it.
