I can also review them with you if you have a new foster dog and would like a refresher....its easier and less painful that way. :) Adoption Terms Conditions and Adoption-Alters Addendum – paperwork you’ll have adoptive homes sign AzBCR vet services – specifies what services are covered for foster dogs and who to contact to arrange vet care. This is for routine vet care only. For a sick or injured dog, please refer to the Foster Home Guidelines document Foster Dog Checklist – this is the checklist of intake items that you’ll work on in conjunction with your Dog Manager Foster Dog Questions – questions about your new foster dog. Please fill this out and sent it to your Dog Manager. This will help the adoption team find appropriate matches for your dog. It can also be helpful in preparing your foster dog’s bio. Foster Home Guidelines – this is the most important document to familiarize yourself with. It contains information such as: • What to do if your dog is having a medical problem or emergency • What to do if you foster dog becomes lost or escapes • What to do with any paperwork you receive when you get your foster dog, take them to vet, etc. This is how we track what services the dogs need and keep track of our expenditures. • Key contact information for the rescue (Board members; who to contact for training and behavior issues; volunteers with microchips; etc.) Names - Retired – As the first foster home of a new dog, you get the honor of naming them. We ask that you avoid the names on this list. It helps eliminate confusion with paperwork and record keeping. Reimbursement Form – turn this form in, along with receipts, when you have an approved expense for reimbursement
