Fear of pain, even without actually feeling pain...just the FEAR of it is enough to create and sustain movement deficits and compensations even after an injury is completely healed and pain is completely gone. This is one of the reasons people need to lift weights, real weights (over time, as they build up), in their rehab. They need to be in a safe place, exposed to a movement that scares them but doesnt elicit pain or damage, and guided through safely, eventually at loads that exceed what they will encounter in real life. You cant expect to fully recover from an injury until that fear is gone, strength is recovered, and confidence is gained.
