Federal Parent/Pupil Survey Forms have been sent home with all - TopicsExpress

Federal Parent/Pupil Survey Forms have been sent home with all students. Parents/guardians should fill in the information requested, sign the form, and if the child is an elementary school or special center student, have the child return the survey to their school. If the child is a secondary school, home teaching schooled, non-public schooled, or Infant/Toddler program student, parents/guardians will be receiving a self-addressed, stamped envelope to be returned to the school system via the US Mail. The U.S. Department of Education has now requested a separate survey form for each child and each survey form must be signed and dated by the parent/guardian. Federal impact aid provides approximately $2 million annually to the school system in direct support of our students.
