Fill corresponding forms. 3. Upload Scanned Copy of Photo (JPEG Less than 50 Kb) Address Proof and Age Proof (PDF) 4. Print ERO/AERO copy of the Acknowledgement generated after Submission. 5. Send it to the Address mentioned in the Print out. Take Volunteer help:: 2. Fill in your personal details in the attached word document and send an email to vfbi.domlur@gmail, along with scanned copies of following documents i. Passport size Photo ii. Address Proof iii. Age Proof (optional, please read below) 3. Please mark subject of email as "VoterID registration: ()" Example : VoterID registration: Himadri Sengupta(redBus) List of acceptable documents for Address proof: i) Bank / Kisan / Post Office current Pass Book ii) Applicant’s Ration Card / Passport / Driving License / Income Tax Return filed or Assessment Order iii) Latest Water / Telephone / Electricity / Gas Connection Bill for that address, either in the name of the applicant or that of his / her immediate relation like parents etc. iv) Postal department’s posts received / delivered in the applicant’s name at the given address. NOTE: If you submit Ration card as proof of address, it should be accompanied with an additional proof from the above list List of acceptable Documents for Age proof: i) Birth certificate issued by a Municipal Authorities or district office of the Registrar of Births & Deaths or Baptism certificate; or ii) Birth certificate from the school (Govt. / Recognised) last attended by the applicant or any other recognised educational institution; or iii) If a person is class 10 or more pass, he should give a copy of the marksheet of class 10 , if it contains date of birth as a proof of date of birth; or iv) Marksheet of class 8 if it contains date of birth; or v) Marksheet of class 5 if it contains date of birth; or vi) Indian Passport vii) PAN card viii) Driving License ix) Aadhar letter issued by UIDAI x) A declaration in prescribed format given in Annexure-1 (made available on demand by registration authorities) made by either of his parents if the person is not educated till class 10 (In those cases where parental declaration is given as proof of age, the applicant will have to be present himself for verification before BLO/AERO/ERO); or xi) A certificate of his age given by a sarpanch of the concerned Gram Panchayat or by a member of the concerned Municipal Corporation / Municipal Committee, in case the person is not educated till class 10 and both the parents are not alive.
