Goals in the management of patients with pituitary adenomas: There are number of goals in the management of patients who harbor pituitary adenomas that are universal regardless of the underlying tumor type. In general, these include: • Eradicate the tumor • Control the tumor if not able to eradicate it completely • Restore hormone hypersecretion to normal via surgery or medical therapy • Replace hormone deficiencies • Assess for associated diseases (MEN-1, complications of hormone hypersecretion, etc.) • Treat disease associated comorbidities (complications of illness such as hypertension, diabetes, arthritis, sleep apnea, osteoporosis, hypercholesterolemia , psychological issues, etc.) Hopefully, achievement of these goals will restore health. I must say, however, that not all patients will have a complete return to normal or desired health because not all of the manifestations of the underlying disease processes are reversible. For example, the arthritis of acromegaly, once it has been initiated, may be improved but not stopped in its tracks with successful treatment of acromegaly. Patients with Cushings syndrome often have lingering mild depression and memory problems. There are plenty of other examples that are beyond the scope of this report.
