Malatesta and William B. Christel, officers of Advanced Waste Services, relative to the premises at 4212 W. Highland Blvd AKA 3801 W. McKinley Avenue from an order of the Commissioner of Neighborhood Services (10154141/Adsit) to obtain a high-hazard occupancy permit for storage and to provide code compliant storage use and handling of class IIIB and corrosive. as required in sections 200-42-2 and 214-3 of the Milwaukee Code of Ordinances. D) Is the nuisance odor emanating from the sewer? a) If so, what is the closet sewer basin? b) Describe the odor, what does it smell like? 3) Commercial administrative staff needs to forward complaint information immediately to the supervisor: A) Mike Mannan (office) 414-286-3975, if no contact, leave message and call (cell) 414-708-9183, no contact leave message and contact next on the list. A Commercial inspector will be sent, along with the Health Department, to investigate the location of the odor nuisances and evaluate immediately. If Verified: A) The inspector will document and will then contact Jeff Dean 414-397-2266 and conduct site visit at AWS. B) The inspector will document findings at AWS, 5) Commercial will contact the Supervisor who will update the Commissioner, Health Department, Alderman, complainant and other departments as needed. B) Commercial Supervisor/Inspector will document findings at AWS, C) Commercial will update complainant on status of complaint immediately by phone call. 5) Commercial Supervisor will contact DNS Commissioner, MHD, DPW, and Alderman’s office with update to complaint on the following business day.
