HOW MUCH... L-Methyl Folate is needed? L-Methyl Folate can be purchased over the counter in dosages as low as 200 mcg, 400 mcg, 800 mcg and 1 mg [1,000 mcg]. 1 mg [1,000 mcg] is NOT a lot of L-Methyl Folate UNLESS you also have several of the Catechol-O-Methyl Transferase [COMT] Single Nucleotide Polymorphisms [SNPs]. The majority of my patients with MTHFR C677T [+/+] and MTHFR C677T/A1298C are taking one of the following: 1. Metanx...which contains 3 mg of L-Methyl Folate, B6 and B12 2. CerefolinNAC...which contains 6 mg of L-Methyl Folate and B12 and NAC 3. Deplin...15 mg of L-Methyl Folate Actually...it is the minority of my patients who cannot tolerate 3mg, 6 mg or 15 mg dosages of L-Methyl Folate and most of those who cannot tolerate these doses are the people who happen to also have several Catechol-O-Methyl Transferase [COMT] homozygote [+/+] snps. The individuals who are the most sensitive to Methyl donors are those with homozygous COMT [+/+] snps and do NOT have any Vitamin D Receptor Taq [VDR Taq] snps. But, if you have not had your 23andMe genetic testing done, then by all means be CAUTIOUS because you do not know for sure that you do or you dont have any of the COMT mutations. So, if you do not know your genetic status it is wise to start on low dose over the counter L-methyl Folate and progress upward slowly with the goal of getting you HOMOCYSTEINE levels down between 6-7. PICTURE: ELEVATED HOMOCYSTEINE MONITOR….HOMOCYSTEINE LEVELS L-Methyl Folate [5-MTHF] provides the methyl group that will eventually help convert HOMOCYSTEINE into Methionine. Ideal Homocysteine = 6-7 High Homocysteine > 10 It is common for people with heterozygous MTHFR C677T [+/-], homozygous MTHFR [+/+] and compound heterozygous MTHFR [C677T/A1298C] to have elevated HOMOCYSTEINE levels. Elevated HOMOCYSTEINE is associated with: Osteoporotic bone fractures Negative impact on connective tissue development Increased blood clotting risk…vascular dementia, pregnancy loss Increased risk of atherosclerosis...vascular disease Neural apoptosis [death] and dopaminergic dysfunction Aberrant DNA methylation Mitochondrial dysfunction Oxidative stress NMDA receptor over stimulation
