History reveals that he began with a mild sore throat about 3 days ago and a low grade fever the lasted 2 days. His mother stated that his breath is “horrid” even after using mouthwash. Examination: weight 158 pounds, height 69 inches, B/P 118/70, pulse 58, and temperature 102.5 °F; Eyes with somewhat of a mildly red conjunctiva; Clear nasal discharge; bilateral tonsillar exudate, erythematous throat, white coating noted over tongue, and few palatal petechiae; tender, mobile, posterior cervical lymph nodes; Chest clear to auscultation; heart rate regular; abdomen soft and mild hepatosplenomegaly noted; The remainder of the exam was normal.
