Hay all you parents out there who dont want your child vaccinated for any reason... Check this out... It is a form to bring to your childs doctor... The only way you will allow your child to be vaccinated is if he/she is willing to fully fill out the form and sign in... Basically saying they (the doctor) believes it is safe and a good idea for you child to be vaccinated, and they personally guarantee your childs safety in being vaccinated....
