Health Alliance Medical Plans is in search of Medical Management Case Coordinators for our Urbana, IL and Springfield, IL location. This position provides case management services to members seeking to maintain wellness, at risk of developing chronic conditions or using high dollar resources, and/or complex or chronic health conditions; provides support to members during transition from hospital to home; functions as a facilitator of interdisciplinary collaboration and care continuity across care settings, coaching the members and care giver(s) to play an active and informed role in care plan execution; and abides by the scope of practice outlined by the Nurse Practice Act and/or other professional affiliation. Requirements include successful completion of an accredited school of nursing, BSN preferred. Must be a Registered Nurse in the State of Illinois. Three years minimum acute care nursing experience in addition to one year utilization Management or Case Management experience preferred. Candidate must possess excellent communication, organization and time management skills. Ability to assess situations and make appropriate decisions based on benefit plans, Medical Policy and Procedures, and the most cost effective care at the most appropriate setting.
