Hi everyone I have been a hygienist for almost 5 years, I am the only hygienist in my office which can be hard when difficulties come up. Sometimes it would be nice to have another hygienist in the office when I have questions or need a second point of view and I dont want to talk with the Dr. about it. I have been reading a lot of different post today and came to the realization that I am way to nice and I let the pt get away with telling me what they want done instead of being more stern with them about how important their Tx will be. For example I have pts that have pocketing mostly 4-5mm in the posterior areas and I talk with them about what needs to be done and they will say that they will bump up their home care, I proceed to tell them what they can do for home care and hope that they will actually do it, or I get grumpy people that dont want to hear it and dont really care, or people that refuse x-rays or refuse restorative work, the list goes on and on. I do and say what I can and document the conversation. I know that I have a responsibility to every pt and my license is on the line. I need some advice on how to be more aggressive with my pts but be nice about it at the same time. I feel like my office is expecting me to have my own protocol on all hygiene situations and I admit I look at every pt differently but it would be nice if I had a base line to go off of especially for the in between cases where they are mostly in good health but have some posterior pocketing. I know I should have this figured out but some days between knowing what the pt want, what the dentist wants, what the insurance will pay or not pay and keeping the front office happy I am about ready to throw in the hygiene towel.
