Be sure to ask yourself these questions before completing the required document: •Is this service related (poor haircut or coloring)? If it is, The Ohio State Board of Cosmetology does NOT have jurisdiction to enforce sanctions on service related complaints. Contact the Better Business Bureau (BBB) for advice. •Are you willing to sign the complaint and provide a phone number so we can contact you? Please remember that all complaints are subject to public records law. •Have you approached the Manager of the establishment to try and resolve your issue? If you bring this issue to their attention, perhaps they can resolve it without Board intervention.
