I met Gretchen, a sweet young woman who needed breast cancer surgery. Gretchen had absolute faith that she was receiving care in a top quality medical center. When i asked her why she choose this particular hospital for her procedure, she explained that the hospitals home page displayed the logo of its prestigious affiliate university saying its center had a number one ranking. My ears perking up over a ranking. When i got back to the office to look it up, i found that this hospitals administration had simply market itself as having a comprehensive breast cancer center. What? Comprehensive? Center? Nothing at this comprehensive cancer center was coordinated, established or noteworthy. It takes an army of dedicated radiologists, oncologists, breast surgeons, and genetic counselors to create a true state-of-the-art comprehensive breat cancer center. This hospital did only a few dozen breast operations a year. A true centers perform hundreds of procedures to get recognition as a center of excellence. They should be ahead of the curve in utilization of effective new technology. They should offer participation in national clinical trials, along with state of the art plastic surgery. As Gretchen prepared for her breast-removal procedure, i couldnt help but see that this hospital didnt even have the equipment to perform any of the newer minimally invasive techniques. Even more worrisome the plastic surgeons didnt know how to do DIEP flap, an operation with well established superior outcomes commonly performed in cancer centers. Gretchens hospital didnt bother to tell her that she could have had this superior treatment at another hospital with a better surgeon a few miles away. If she had simply known the right information to be an educated consumer of medicine, she could have driven those few miles away to a better outcome. Instead she stayed where she was, and her reconstructed breast came out looking permanently unnatural. When i asked her if she pleased with the result, she said, I dont really know what to compare it to, but yes, I feel very blessed. (A story from Marty).
