Jen, perhaps Spencer can jump in if I have this wrong, but what is a really concerning feature is NORMALLY if we have a hypocaloric state we see lipid improvements almost regardless of composition. Especially if this is accompanied by weight loss. What is being described here is not only low cals AND weight loss, but still dramatic increases in LDL-P. Now, we DO tend to see decreases in inflammation, coagulation panels ,which might off set the proposed gradient driven process of elevated LDL-P, but then again, perhaps not. Some of the allure of the coconut oil and butter is the MCT content which should drive ketosis more potently...but it also appears to drive Lp(a) and total LDL-P. Mark Shields- Nice find on the keto-med paper. I do not think there is any doubt a KD has huge therapeutic potential for a variety of issues, and might even be a sweet spot for some folks, but we might need to rethink the substrates going into that. We might need to rely on ketone esters or similar products for say athletes to offset the lipid changes being observed.
