Just go to the El Casino Ballroom and register as a walk-in. El Casino Ballroom is located at 437 E. 26th Street, 85713, between 2nd and 3rd Avenue. The enrollment times are from Noon to 3:30 p.m. You will fill out a survey questionnaire, have a waist measurement and have a blood draw, then complete another survey at home. The process on enrollment day has been averaging just 30 minutes per person, so it won’t take up a large portion of your afternoon activities. Thank you.
