In a normal world, every single person with an iota of responsibility in a case like this would be forced to resign or be fired, from the chief of police to the cop who threw the grenade to the detectives or investigators who got the wrong house. If the victim was a person of status there would be such pressure, careers would be ended, liability would be widely assigned, and things would happen. But we live in a quasi-police state where the culture of fear has caused our society to drift (apologies to Rachel Maddow) to a place we really do not want to be.
