IowaHealth+ Approved as Medicaid ACO!!! IowaHealth+ has received approval from Iowa Medicaid as an Accountable Care Organization (ACO) to encourage healthy behaviors and improve health outcomes for thousands of Iowans on the Iowa Health and Wellness Plan. IowaHealth+ is a network of eight Federally Qualified Health Centers (FQHCs) serving all who walk through their doors, regardless of ability to pay or insurance status. The IowaHealth+ network provides patients with a continuum of primary and preventive services that helps them stay healthy. These services include medical, dental and behavioral health, as well as supportive services such as health education, transportation and interpretation. “We are very excited about this announcement,” said IowaHealth+ CEO Ted Boesen. “The IowaHealth+ ACO allows our health centers to provide care within an evolving Medicaid system and remain leaders in offering individualized, quality health care to thousands of Medicaid patients. We are pleased Iowa Medicaid has recognized the value of this unique ACO, which allows us remain focused on our collective mission to provide accessible, affordable health care for all.” ACOs are groups of doctors, hospitals, and other health care providers, who have agreed to provide integrated care to patients, with the goal of achieving better health results for Iowans, as well as fostering accountability in their care. Under these agreements, health care providers assume financial risk to deliver high-quality outcomes of a defined patient population - in this case, Iowa Wellness Plan members. ACOs are incentivized by quality bonuses that align with the Iowa Health and Wellness Plan’s Healthy Behaviors program. Under that program, members’ monthly contributions are waived if they complete a risk assessment and annual physical. If the activities are completed in 2014, the monthly contributions would be waived in 2015. Members of the IowaHealth+ ACO include: • All Care Health Center (Council Bluffs) • Community Health Centers of Southeastern Iowa, Inc. (West Burlington, Columbus City, Keokuk) • Community Health Centers of Southern Iowa, Inc. (Leon, Lamoni, Albia, Centerville, Chariton, Corydon) • Crescent Community Health Center (Dubuque) • Peoples Community Health Clinic (Waterloo, Clarksville) • Promise Community Health Center (Sioux Center) • River Hills Community Health Center (Ottumwa, Centerville, Richland) • Siouxland Community Health Center (Sioux City)
