JOB OPPORTUNITIES Community Co-ordinators in Health and Social Care (£31,160 per annum pro rata) Interlink are recruiting 5 Community Co-ordinators. These posts are supported by the Intermediate Care Fund from Welsh Government to encourage greater collaboration between Health, Social Care, Housing and the third sector, for the benefit of our frail/elderly population. The Community Co-ordinators will work in localities within Merthyr Tydfil and Rhondda Cynon Taff and will support individuals, communities and organisations to work together to reduce social isolation and support older people to maintain their independence. Interlink are seeking individuals with a proven track record of working collaboratively, and who are committed to enhancing individuals’ lives and finding solutions. These full-time positions are offered on a 9 month fixed-term basis. They will be based within different localities throughout Cwm Taf and will be hosted by Voluntary Action Merthyr Tydfil and Interlink RCT.
