KIDNEY WEEK PHILADELPHIA 2014 CHOLEMIC NEPHROSIS=BILE ACID NEPHROPATHY Dr. Andrew Chang revealed that a new histopathologic entity has been increasingly seen in cirrhosis patients. Cholemic nephrosis, or bile acid nephropathy, is an accumulation of bile acids in the tubulointerstitium seen commonly in jaundiced patients. In a recent article (KI 2013; 84:192) it was hypothesized that bile acid nephropathy may be the cause of renal dysfunction that is seen in the hepatorenal syndrome (HRS). Even more interesting is the theory that bile acid nephropathy is **irreversible**, making many in the audience wonder if there is a point in the course of HRS when liver transplantation would not restore kidney function. This is a new entity and more data is needed but we should stay tuned to cholemic nephrosis as it could have major implications in how we look at kidney disease in HRS.
