(a) This offer of provisional admission is valid till Monday, September 16, 2013 (b) At the point of your registration, the original of the credentials you listed in your application form will be verified. (c) If at any time starting from the registration, it is discovered that any of the credentials you claimed to have had is false, forged or incorrect, this offer of admission would become invalid and you would forfeit all fees paid in respect of same. (d) The names by which you are admitted and by which you will be registered will be the names which will appear on the statement of result that may be issued to you on the successful completion of the Programme except a change of name for female candidate who got married during the course of the Programme. (e) The Pre-Degree Science Programme is not an undergraduate Programme. Therefore, this admission letter does not make you an undergraduate of Ladoke Akintola University of Technology, Ogbomoso. (f) Placement into the undergraduate Programme will be based on your level of performance in the Pre-Degree Science Programme, successful completion of the Pre-Degree ICT Training scheme and availability of space in the undergraduate Programme. (g) You are to note specifically that: This offer of admission is valid for only one contact session. Fees paid are non-refundable. The minimum entry qualification for the Progaramme is credit at O’ level in each of the following subjects: English Language, Mathematics, Physics, Chemistry and Biology. (h) You are to obey all the rules and regulations as specified in the PDS hand book, on matters such as acts of examination malpractice, violence, looting, vandalism, secret cults or any form of brigandage e.t.c. You are to show at all times respect to the Office and person of the Vice-Chancellor and other University Officers. Failure to comply with this provision shall lead to your automatic expulsion from the Programme. 2 The Programme fees, which should be paid online are as stated below: (i) Acceptance fee (to be paid by every successful Candidate) N15, 000:00 (ii) Tuition fee Oyo and Osun State indigenes N100, 000:00 (iii) Tuition fee for students from other states N115, 000:00 (iv) ICT Fee N11, 500:00 3. (a) If you accept this offer, proceed as follows: come to the PDS complex, LAUTECH, Ogbomoso as from Monday, August 19, 2013 to collect permit for payment of Fees with the following documents: (i) Evidence of Payment of Acceptance fee online. (ii) A copy of this letter. (iii) A Photocopy of O’level result(s) (iii) NECO, WAEC or NABTEB scratch card(s) as appropriate for authentication of your O’level result on the internet (b) Payment of other fees commence on Monday, August 19, 2013 and end on Friday, September 13, 2013. However, payment of acceptance fee online commences immediately for all successful applicants. (c) Procedure for Payment: Only applicants who have been issued clearance certificate after payment of acceptance fee online and authentication of their O’ level results on the internet should proceed to LAUTECH website: lautech.edu.ng to make payments such as (Tuition and ICT Fees) online using Interswitch enabled Debit Cards. (d) Medical fee of N1,400:00 to be paid to the Resident Accountant in the PDS Complex 4. When reporting for registration after the completion of all payments, bring along with you the following documents: Receipts of Payment of Acceptance fee, Tuition fee, ICT fee, Medical fee, two (2) recent Passport Size Photographs, completed State Identification form duly signed by the Chairman and Secretary of your Local Government Area, a copy of your letter of admission , a copy of your entrance examination score sheet, clearance certificate issued after authentication of O’ level result(s), letter of attestation from the Principal of your former secondary school and from your religious cleric. 5. Please, note that you must sit for JAMB/UTME during the course of programme. Also, note strongly that your placement into 100 level will be determined by your scoring a minimum of 250 marks in the PDS Final Examination, obtaining the national minimum pass mark in the UTME prescribed by JAMB and successfully completing the mandatory Pre-degree ICT Training Scheme. 6. Lectures commence on Monday, September 16, 2013. No registration will be allowed after Monday, October 7, 2013. 7. Please note that the PDS mid-programme test and final examinations will be computer based. 8.
