Liddle’s syndrome – the important findingsare hypertension and hypokalaemia. There is suppression of the reninaldosterone system. The mode of inheritance is auto somal dominant with variable penetrance, and the defect is in the gene encoding the epithelial Na channel (ENaC) on chromosome 16. The hypertension responds better to amiloride, which directly blocks ENaC. Spironolactone acts by regulating aldosterone and Liddle’s syndrome does not respond to this regulation. Liddle’s syndrome can mimic Conn’s syndrome and this is a differential diagnosis.
